Forms

Create your own forms for onboarding and check-ins, send them to your clients, and track progress with data points.

Updated 20.10.2026

How to create your own forms

  1. Go to Settings and locate the forms section. You can also start with a template from the public library and customize it.

  2. Give the form a name (e.g., "Online check-in").

  3. Add steps or topics (e.g., health, exercise, diet, lifestyle).

  4. Add questions to each step. You can choose from:

    • Text field (a single line or a larger text area)
    • E-mail
    • Radio buttons or drop-down lists
    • Date
    • Image upload (before/after photos, injuries, etc.)
  5. Connect questions to data points to track progress over time (weight, body measurements, body fat, motivation, pain level, stress, energy, sleep, etc.).

  6. Mark questions as mandatory by enabling Required.

How to deliver forms to clients

Via products

Link an onboarding form and/or a standard check-in form to a product. The client will receive the form automatically when the product is activated.

Automatic reminders

Set up check-ins for a specific day and time to send the client a push notification via the app. The client will also receive the form via e-mail. Only one automatic form can be active per client at a time.

Manual distribution

Send a form directly to the client from their client card:

  • Fill it out together with the client during a consultation.
  • Send a link for the client to complete in their browser.

Managing form responses

  • Find all responses under Reports on the client card.
  • View an overview of sent, completed, and incomplete forms.
  • Copy data to data points to display progress graphically on the client's progress page.
  • Edit responses if the client has provided incorrect information.

Create custom forms tailored to your practice and use data points to track progress over time. Forms that are no longer in use can be deactivated or deleted.
